The envelope I write Sir Anthony Hopkins, yes, I know. But, the letter, what I write "Dear Sir (no name)" or "Dear Sir Mr. Hopkins" or "Dear Sir Hopkins"? I don't know, what I write? I know always "Dear Mr. oder Mrs.". But "Sir", I don't know??? I hope you understand.

Say "Dear Mr Hopkins" or "Dear Sir Anthony Hopkins" its OK, whatever you put there is no etiquette.

When writing to a Sir you usually write the knight's given name or full name, but not with the surname alone. This would mean either "Dear Sir Anthony Hopkins" or "Dear Sir Anthony".
I'm sure your 'Mr Hopkins' will do as well and good luck to you I sent my request last week, but I used the Shepperton Sudio's address.
