On 11/23/12, I sent a letter, 2 cards, and SASE to the former quarterback at:

Jeff George
Jeff George Foundation, Inc.
1908 Schwier Ct.
Indianapolis, IN 46229-2154

On 12/10/12 I received my cards back, personalized and signed in blue sharpie! He also included an extra signed card of him on the Colts and on the back he wrote, "Be Confident! Always give 110%! -Best Wishes- Jeff George #11." Great success! Sorry, no scan.
