Suggestions on ways to place address plays.

I want to send some letters out to celebrities on Broadway but I do not know the right way to address the letter. Do I put the persons name first then the plays name than the theater name? Do I leave the plays name out? Any help would be greatly welcomed.

Celeb name
Play name
Theatre
