On 12/1/12, I sent a letter, 2 cards, and SASE to the former MLB utility player at:

1682 N 1950 W
Provo, UT 84604

On 12/18/12, I received my cards back, signed in blue sharpie. Sorry, no scan.

Congratulations on your success, I was also fortunate enough to get his signature through fanmail a couple of years myself. :D
