On September 23, 2011, using the database address:

Phyllis Diller
163 S Rockingham Ave
Los Angeles, CA 90049-2513

I sent LOR, SASE, and 1 8 1/2x11 b&w photo. Today, October 7, 2011, I received my photo back signed and personalized as well as a second photo she included, also signed and personalized. Ms. Diller is a true comic legend and I am really happy with this success and her thoughtfulness including a second picture!
