On July 12, 2011, I sent a LOR, 4 x 6 picture, and SASE to database address:

Polly Holliday
The Blake Agency
23441 Malibu Rd
Malibu CA 90265

Today, August 23, 2011, I received my picture back, personalized, and signed in ink. I am thrilled. I tried 5 years ago to get a picture to no avail but then I only sent LOR and SASE.
