After contacting Garry Kitchen by email (http://www.garrykitchen.com/) he sent me a mailing address. He also informed me that he continues to work with David Crane. Both were game creators for Activision. Garry also programmed Donkey Kong on the Atari 2600 for Coleco. David Crane programmed A Boy and His Blob for the Nintendo Entertainment System after leaving Activision. I sent LoR, return postage, SAE, and photos on Aug 14, and received everything signed on Sep 26. Mr. Crane also included a pin as you can see by the postage in the top right corner.
