First picture is the autograph and the second is the thank you note in my SASE. I included a $5 money order made out to St. Jude's Children Research Hospital. If you go to joetheismann.com, it'll tell you all the prices he charges for autographs (and they're pretty reasonable compared to a lot of other players). As noted in his webpage, expect a 2-4 week delivery if everything is correct, though mine took just 8 days.
