I got lucky a second time. I sent him a LOR, photo and SASE on 2/15/15 to this address:

Los Angeles Dodgers
Camelback Ranch - Glendale
10710 W. Camelback Road
Phoenix, AZ 85037

and got it back signed today (2/8/16). I tossed the envelop before I took the picture.
