Sent a LOR, SASE, a DVD slipcover, ticketstub, and photo on 09/23/2014.
Received my items signed on 10/27/2014.
Used a private address, but the return address may be a working address.

I just realized, I also included a Brian Wilson CD cover, but that didn't make it's way back. Not complaining at all. Just forgot to mention.
