Emailed GRRM via his site and got a reply from his PA Patricia asking for an SAE sent to:

I did that with $2.00 US to cover postage (the amount she suggested back to the UK) around 2nd October. Today I got home to find not only a signed bookplate (which I'd requested) but also a signed pic from GRRM. Thrilled with the response.
