Sent a LOR, SASE, and TC to :

Jack Clark
1659 Mount Vernon Dr
Saint Charles, MO 63303

Sent : 10/18/17
Received RTS : 5/17/18

Came back with a letter from St. Louis Sports saying that they take care of Jack Clark's autograph request, with their address and charges.
