How did you know where to send this? I want to send my book, but I'm somewhat hesitant for a couple of reasons, but I just want to make sure this address has been used before a few times before I take a risk.

If your concerned go ahead and write him a letter asking if you could send the book or the book jacket for him to sign. If he says yes make sure that you send a copy of his reply along with the book or jacket.
