How do I adress a letter to a british knight?

I want to send a letter to the British knight and actor Sir Michael Gambon, but as a German I do not really know how to adress it.
How must my envelope look like?
For example, do I write on the envelope:

"Sir Michael Gambon CBE,
(adress)"

?

and how do i write the salutation in the actual letter:

"Dear Sir Michael Gambon CBE"

?

I would be very grateful for an answer :D [germany] [uk]
