It's been a while, sadly. First I recall, though probably not the earliest, was in a book from 1996 by Mark Allen Baker. Baker would write stars and publish his successes with the addresses and a few pictures. Van Dyke's secretarial was pictured in the 1996 book though it was unknown at the time they were fake so it was listed as a success.
