As for your question, I would send the request to the address which is in the city where she spends most of her time, which I am guessing would be Nashville. If you did send it to her representation in California, it would probably be forwarded to her in Nashville, but writing directly to her would take less time.

I have signed up for IMDB Pro and I have a question. Would I use the address for CCA in Los Angeles, CA or Nashville,TN for a country music star. For example Shania Twain would I use the one Posted that is Los Angeles or would I use the Nashville address?
