Testament failure RTS

I got the package with a letter of request plus a cd booklet returned to me today. I sent it in early October. Had a return to sender sticker on it saying "unable to forward." I don't have a picture as I managed to get the sticker off and I reused it (with a new label over the one to Testament) for another request. The address I used was the one that is on this site.
