Well, it took about 9 months to get it, but well worth the wait. On 05/30/2012 I sent a letter, SASE, and 2 disc covers to:

Received both disc covers today (02/26/2013) signed and 1 personalized. Clerks is one of my favorite movies, so I was very happy to see these in the mail!
