Sent this great Hall of Fame Golfer an 8 by 10 back on 3-4-11 and recieved it back on 5-11-11. I also recieved a letter from Jonelle Jaimes, she must be his agent or something, stating that "...this request fulfills the maximum number of autographed items I may recieve." I guess his autograph policy is that people can only 1 autographed item request per household. Anyway, here's the address I used:
