Sent rookie card in toploader, SASE, short note, and $25 dollars cash to Gaylord Perry at:

P.O. Box 489
Spruce Pines, NC 28777

Sent on April 26th, 2011. Received back in SASE on May 11th, 2011. Card was in toploader and undamaged. Signed in blue sharpie. I didn't have a price list but I gathered from this site and others that he charges $25 dollars for a baseball card. SASE was postmarked Ashville, NC on May 9th.
