Sent a hand written letter to Mr. Carpenter, 3 DVD sleeves, a SASE some time around the 14th or 15th of November, 2012. I received back all 3 sleeves personalized on 12/12/12. I used the address provided and his people were nice enough to pitch in for the remaining $.40 for postage. Happy hunting!
