Jeff Hornacek - Ex NBA Player (Utah Jazz) - February 21st, 2013
I mailed 3 cards, letter, and SASE to Jeff Hornacek and received back 2 cards signed. The 3rd one I believe he did not sign because the card had him and John Stockton in it and was just a basic Jazz card not for any particular player. I am fine with that and just happy to get the other 2 back.

Sent: emailed from her website. (Received email response from her manager providing physical mailing address) LOR, a CD booklet and SASE sent on 10/23/12
Received: CD booklet inscribed, personalized and signed, along with a signed note and a sealed copy of her brand new CD in a bubble mailer I did not send, although they kept my SASE, on 11/16/12

On 1/20/12 I sent a letter, card and SASE to Jeff Hornacek who played for the 76ers, Suns and Jazz during a great NBA career. On 10/30/12, I received the autographed card. I sent it to the Utah Jazz addy in the database since he is an assistant coach for them.

On 8/9/12 I sent a photo, letter, SAE and IRC to John Stockton at the Utah Jazz address.
On 9/24/12 I received the photo back signed and IRC in envelope
