If you live in send a properly stamped and self addressed envelope (minimum size 8.5" x 4") with your request letter and a photo. You can include a piece of cardboard to keep the photo from bending in shipping and also add "Do Not Bend" on the envelopes. Send your letter and wait. 6/29/2005 - As you may know Mickey Carroll is the last living Munchkin from The Wizard Of Oz. At least the last one with a speaking part. There are others still alive, but they were just extras, with no speaking part. Mickey Carroll was the Mayor and if I am not mistaking also one of the Lollipop Guild. I obtained mine in person at, believe it or not, a sports card show. What are the chances that he'd be walking around the show. Well he is a really nice guy and his address is even in the local phone book. The address is-
