If you live in USA send a properly stamped and self addressed envelope (minimum size 8.5" x 4") with your request letter and a photo.
You can include a piece of cardboard to keep the photo from bending in shipping and also add "Do Not Bend" on the envelopes.
Send your letter and wait. On average, there is going to be a 3+ month wait for a response.

Trevor Wilson (Frmr SF Giants pitcher) success - January 7th, 2013
On 10/12/12, I sent a letter, card and SASE to Trevor Wilson who pitched for the San Francisco Giants. On 12/28/12 I received the signed card. Unfortunately, he substituted another card for the card I sent. We were trying to get the 1989 set signed, but its still a fun success. The addy I used was 11857 White Lane, Oregon City, OR 97045
